Step 2: Pay Fees

To pay any state required fees, contact (915) 521-7690, option 2 - Release of Medical Information.

  • Show proof of identity (Copy of government issued photo with name)
  • Fax completed form to (915) 521-7591
Step 3: Mail Your Request

Your request will be processed and mailed to address on the authorization form, or you may call to arrange pick up.
