Please include the following information in the application:

  • Letter of Intent describing interest in the PGY1 Community-Based Pharmacy-Residency Program
  • Curriculum Vitae
  • 3 References
    • One from a faculty member or preceptor
    • One from an APPE ambulatory/community preceptor of direct patient care
    • One from employer or other pharmacy professional
  • Extracurricular Information
  • Basic Spanish language competency highly encouraged
